Template help needed

If anyone has the time and skills to configure this template with multiple "if" parameters for each level of qualification (listed here), it would be greatly appreciated. Ideally a parameter for every level of qualification from this:

to the above template would be employed, to save using so many templates for different levels of qualification. Another "if" parameter for users having achieved the service award. Ask User:Pumpmeup for details if needed, or be bold.
