- yearly until age 6
- every other year until age 11, then yearly until 18
History
- pregnancy/neonatal history
- feeding/diet
- immunizations
- developmental assessment
- growth, energy, appetite, sleep and review of systems
- past medical history, medications, allergies, family history and social history
Physical Exam
- Growth parameters:
- serial height
- weight
- head circumference (until age 2)
