Pantoprazole

General
  • Type: Proton-pump inhibitor

Adult Dosing

Upper gastrointestinal bleeding
  • 80mg IV x 1; then 8mg/hr OR 80mg IV BID

GERD, Peptic ulcer disease, Esophagitis
  • 40mg IV or PO daily

Pediatric Dosing
  • <5yo: safety/efficacy not established
  • >5yo
    • 15 kg to <40 kg: 20 mg PO qDay
    • 40 kg or greater: 40 mg PO qDay
